• Preferred Masjid (Location):
  Which of our masjids would you like to host your ceremony?

• Preferred Officiant (Sheikh):
  Do you have a specific Sheikh you would like to perform the Katb Kitab/Nikkah? If so, please provide their name.

• Civil Marriage Registration:
  Has your civil marriage been completed?
  Is it registered in the state where you reside?

• Religious Affiliation of Both Parties:
  Are both individuals Muslim?
  If one or both parties are not Muslim, please indicate each person’s religion.

• Number of Guests:
  Kindly share the estimated number of attendees so we can make appropriate arrangements.

• Coffee/Tea Service:
  Would you like us to provide coffee or tea service during the event?

• Food Service:
  Will you be serving food at the event? If so, please mention the type of catering and any special requirements.
